Definition
What telepsychiatry means
Telepsychiatry is the delivery of psychiatric care, including evaluation, diagnosis, and medication management, by secure live video rather than in a physical office. A licensed clinician meets with you over an encrypted connection, takes your history, makes a diagnosis, and prescribes and adjusts treatment, all the work of an outpatient psychiatric visit, conducted remotely. It is one part of the broader field of telemedicine.
In practice telepsychiatry removes the logistics that keep many people from care: the commute, the time off work, the wait for a nearby appointment, the difficulty of finding a specialist in a rural area. You meet with your clinician from home. Much of psychiatric care is conversation and careful history-taking, which translate well to video, and tools like the PHQ-9 and GAD-7 work the same way regardless of location. shrinkMD provides care this way to adults eighteen and older, currently in Maine and Nebraska with more states coming soon.
This matters because the evidence is strong. Studies comparing telepsychiatry to in-person outpatient care find comparable outcomes for diagnosis, treatment response, and patient satisfaction across many conditions. For medication management and most therapy, the remote format does not weaken the care. Clinicians are licensed in the state where the patient is located, which is why a practice expands one state at a time.
A common misconception is that telepsychiatry is a lesser or watered-down version of real psychiatry. It is full psychiatric care delivered over video, with the same diagnosis, prescribing, and follow-up. Another misread is that everything can be done remotely. Some situations, such as a psychiatric emergency or certain medical monitoring, call for in-person care, and a responsible telepsychiatry practice recognizes those limits and refers when needed.
