This Notice at Collection is provided by shrinkMD Health Inc., a Delaware corporation ("shrinkMD"), to California residents under the California Consumer Privacy Act, as amended by the California Privacy Rights Act (CCPA/CPRA). Note: information you provide once you become a patient, and your health records, are governed by HIPAA and our Notice of Privacy Practices, not by the CCPA, to the extent HIPAA applies.
1. Categories of personal information we may collect through this site
- Identifiers, such as name and email address you submit through a form or the waiting list.
- Internet or network activity, such as pages viewed and general device and browser information, collected through cookies and analytics.
- Geolocation (approximate), such as the general region inferred from an IP address.
- Inferences drawn from the above to operate and improve the site.
We do not knowingly collect the sensitive personal information of website visitors beyond what you choose to submit, and we ask that you not submit health information through general website forms.
2. Purposes for collection
To respond to your inquiries, manage the waiting list, operate and secure the site, understand and improve how the site is used, and comply with law. We do not collect information for purposes that are incompatible with these.
3. Sale or sharing of personal information
shrinkMD does not sell personal information, and does not share personal information for cross-context behavioral advertising, as those terms are defined under the CCPA/CPRA.
4. Retention
We keep personal information only as long as needed for the purposes described here or as required by law, then delete or de-identify it.
5. Your California rights
